According to the Security Officer at our delivery location, the Driver backed toward the dock and tried to line up the truck lift gate with the loading dock. The truck was lower than the loading dock and the Driver was unable to adjust the truck lift gate to a higher height. The Driver then tried to move the truck forward (away from the loading dock). The Security Officer warned him that if he did so, the skid would likely fall off the truck. The Driver continued and the skid did fall between the loading dock and the truck. |
